If we have your email address on file you may have received an enrollment email with the subject "Our New Online Patient Tools". Please take the time to create a password and set your communication preferences. We look forward to utilizing this new technology to better serve your needs!

Patient Forms

If you are a new patient to our office, please visit our Patient Forms page to download our new patient registration and health history forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your dental needs more quickly than completing them on your arrival. Payment Options

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

Dental Insurance

We are contracted with Delta Dental Premiere and Anthem Blue Cross PPO-300. We accept all PPO dental insurance. Many of our patients use their PPO dental insurance at our office with little to no out of pocket expense.

Please note; As a courtesy, our staff will submit claims to your insurance company on your behalf. Insurance benefits vary, so it is important that you know and understand your coverage. The amount of coverage your policy provides depends on the plan you participate in. Your insurance may pay all or some of your bill ultimately, the patient is financially responsible.

My husband and I have been patients of Dr. Koo for about 30 years. We plan to remain his patients for another 30 or so - if we make it to 100! Dr. Koo will have to retire before we will change dentists.

I've seen Dr. Koo three times now and my teeth have never felt or looked better! I will say that I've never been more confident and felt more at home than I do with Dr. Koo. I believe that he really cares about his patients, this is a major plus for me considering some of my past dental experiences with other dentists. The first time I went in he repaired some poor dental work I had done by another dentist. This was a big problematic area for me but he accessed the issue and I've had no problem since. Recently, Dr. Koo recommended I explore a whitening procedure (I DRINK WAY TOO MUCH COFFEE) and after I was finished the process my teeth literally have never looked better! Highly recommend this office, front desk staff is super awesome too!

Let me start off by saying that I have been seeing Dr. Koo for over 25 years now and I don't ever plan on seeing anyone else. Over the years Dr. Koo and his staff (from the front desk to his dental assistants) have been courteous, caring, and extremely helpful to my entire family and I. From the latest equipment, to years of knowledgeable experience, there has never been a dental issue that Dr. Koo has not been able to assist me with.

The entire staff is not only friendly but very informative and helpful. They all work together very well to keep the appointments, service, and overall practice running smooth.
